Job Title: Behavioral Health Clinical Social Worker Job Specialty: Clinical Social Worker Job Duration: 13 weeks Shift: Full-time, Day shifts Guaranteed Hours: 30 hours per week Experience: Minimum of 2 years required License: California Licensed Clinical Social Worker required Certifications: Bachelors in Social Work required, Masters degree preferred Must-Have: Three to five years of hospital social work experience preferred Job Description: - Develop post-acute care plans in collaboration with the Case Manager RN for patients with personal, social, or environmental barriers. - Provide interventions for patients with complex psychosocial needs, assistance with eligibility determination for social programs, and qualify for community assistance. - Offer crisis intervention to patients and families with psychosocial needs. - Coordinate and facilitate the development of discharge plans for high-risk patient populations. - Communicate with patients and families to assist in developing an adequate continuing care plan for discharge. - Resolve personal and environmental difficulties that may interfere with a patient's medical care. - Screen, assess, and provide resources for specific social drivers of health needs. - Act as an expert on social health resources available in the community. - Refer patients to Behavioral Health for formal assessment needs and outpatient services. - Provide education and support for food access and housing. - Coordinate the patient transportation program. - Assist families in crisis situations in understanding the implications of medical situations. - Conduct financial assessment and referral services. - Arrange for in-home care and equipment.
