Registered Nurse (RN) | Case Manager Location: Fresno, CA Agency: Lancesoft Inc Pay: $3,185 per week Shift Information: Nights - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/29/2026 About the Position Shift: 8-hour day shift Monday–Friday, 8:00 AM – 4:30 PM 40 hours/week On-call: ~7 days/month Location: Hinds Hospice , Fresno Responsibilities: Provide in-home pediatric hospice care (birth–21 years) Conduct 4–5 patient home visits per day Perform assessments, symptom management, and care coordination Educate and support families Document care in NetSmart Participate in on-call rotation Requirements Requirements: Active California RN License Minimum 2 years pediatric experience (PICU / NICU / pediatric acute care / oncology / hospice) Basic Life Support (BLS) certification Hospice or case management experience preferred Valid driver’s license Reliable vehicle Current auto insurance and vehicle registration Comfortable with home visits and on-call rotation ???
