Job Description:
Job Description Job Description Position Summary The LVN plays a
vital role in ensuring the health and well-being of EPOCH
residents. This position focuses on coordinating patient care,
managing healthcare resources, and supporting treatment adherence.
The LVN will work closely with onsite case managers, healthcare
professionals, and external providers to deliver integrated and
compassionate care. Essential Duties and Responsibilities The
essential functions include, but are not limited to the following:
Coordinate with physicians, case managers, and behavioral health
providers to ensure continuity and consistency in care Participate
in care team meetings and contribute to interdisciplinary treatment
planning Monitor residents for changes in physical and behavioral
health status Support medication compliance and assist with minor
medical needs as appropriate Provide basic first aid and health
assessments, escalating care when necessary Maintain accurate,
timely, and confidential documentation of patient care and
interactions Update treatment plans and medical records using
designated healthcare management systems Educate residents and
their families on medical conditions, medication adherence,
hygiene, and wellness strategies Promote self-management and
preventative care practices Minimum Qualifications (Knowledge,
Skills, and Abilities) Active and unrestricted LVN license in the
state of California Experience working with populations affected by
homelessness, behavioral health, substance use, or reentry is
strongly preferred Strong verbal and written communication skills,
with an ability to work effectively in a multidisciplinary team
Proficiency in electronic health records and healthcare management
systems Ability to assess clinical situations and exercise sound
judgment Demonstrated ability to manage multiple responsibilities
in a fast-paced environment Cleaning driving record Work
Environment: Shelter-based setting with regular client interaction
May involve working evenings or weekends depending on program needs
Must be comfortable working with diverse populations in a
trauma-informed care model
