Personal Care Supervisor POSITION SUMMARY: The Personal Care Supervisor at BoldAge PACE is responsible for overseeing the delivery of high-quality, participant-centered personal care services. This role involves managing personal care staff, coordinating care plans, and ensuring services align with participants' needs and organizational standards. The Personal Care Supervisor collaborates with interdisciplinary teams to develop and implement care strategies, supervises staff performance, and ensures compliance with state, federal, and PACE program regulations. By fostering a supportive and compassionate care environment, the Personal Care Supervisor upholds BoldAge PACE’s mission to provide exceptional care and promote participant well-being. ESSENTIAL DUTIES AND RESPONSIBILITIES: Ensure all Personal Care Specialists (PCSs) meet position-specific competencies before delivering participant care, upholding organizational standards. Supervise PCSs in providing high-quality care as active members of the PACE interdisciplinary team (IDT), offering guidance and support in daily tasks. Conduct regular assessments of PCS skills and competencies to maintain quality standards and identify training needs. Oversee the provision of care in the center, clinic, and participants' homes, ensuring services align with individualized care plans. Manage daily PCS assignments for transportation escorts, day center activities, or home care to meet operational requirements efficiently. Ensure PCSs assist participants with "Activities of Daily Living" (ADLs), including bathing, dressing, grooming, meal assistance, and personal hygiene. Supervise and support PCSs in planning and facilitating group and individual activities while monitoring participants' physical and emotional well-being. Foster strong relationships between PCSs and the IDT to enhance participant care, ensuring seamless communication and collaboration. Collaborate with the IDT to develop comprehensive care plans and ensure PCSs are informed and compliant with participant-specific plans. Fulfill role of other member(s) of the IDT as needed based on competencies and licensure requirements. Ensure PCSs complete all required charting and reporting, accurately documenting participant care and health changes per policy. Review and maintain accurate participant medical records to ensure compliance with organizational and regulatory standards. Adhere to OSHA safety guidelines and standard precautions, ensuring PCSs follow all health and safety protocols. Conduct ongoing PCS training in care protocols, safety measures, and effective communication, promoting professional development and best practices. Organize and lead continuing education sessions, in-service meetings, and staff training to keep PCSs informed of regulatory requirements and advancements. Build positive relationships with participants and families, serving as the primary point of contact for inquiries, communicating health status updates, and resolving concerns with professionalism and compassion. Ensure PCSs maintain clean, safe, and organized care environments in all participant settings. Lead quality improvement initiatives setting goals to enhance service delivery and participant outcomes and actively participate in organizational projects and meetings. Participate in professional affiliations, maintain required certifications, and complete continuing education to stay informed of industry advancements. EXPERIENCE AND EDUCATION: High School diploma or GED required Associate or bachelor's Degree (Healthcare administration, social work, or nursing-preferred) or CNA or CHHA certification required. Two (2) years of experience working in a hospital, nursing home or community-based setting is preferable. 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
